Goal:
Find and discuss issues where the individual is falling short on self-care behaviors that negatively impact the course of Schizophrenia.
Possible Action Steps
- Elicit individual’s concerns, questions and ideas related to the self-management of Schizophrenia.
- Jointly problem-solve to come up with one new approach to mitigate a troubling symptom.
- Together, make a plan to incorporate the new approach to mitigate a troubling symptom into a daily routine.
- Ask individual to commit to trying out the new approach to mitigate a troubling symptom in the upcoming week.
Goal: Identify situations where self-management of Schizophrenia can be further improved. Revisit past goals as needed.
Possible Action Steps
- Together, make a short list of health behavior changes the individual would like to undertake.
- Have the individual select just one health behavior to change.
- Find out what would cause the individual to prevent making a positive health change?
- What would help the individual make one additional health behavior change?
- Have the individual create a plan (with support system or independently) for making one small positive health behavior change.
Goal: Have the individual identify their relapse indicators.
Possible Action Steps
- Have the individualtalk about symptoms that often lead to relapse. Can the individual identify what triggers the symptoms to start in the first place?
- Have individual identify situations or events that are likely to throw them off-track in managing the triggers that bring on symptoms of Schizophrenia.
- Ask individual to identify a few steps they could take to get back on track and create a plan to address relapse (i.e., by accessing family/social support; by contacting their psychiatrist; taking medication as prescribed, keeping up with ADL/IADLs, etc.)
Goal:
Have the individual learn about sound sources of health information in general and Schizophrenia in particular.
Possible Action Steps
- Find out where the individual currently goes to learn about health issues, including Schizophrenia.
- Without disregarding where the individual seeks information, provide a few examples of places where sound health information can be located. Do not rely exclusively on internet sites.
- Create an outline/document of sources of information and ask the individual to share or discuss with their family/support system.
Goal: Have the individual focus on how use of non-prescribed drugs and alcohol can contribute to increased symptoms of Schizophrenia, and impair daily functioning.
Possible Action Steps
- Ask the individual to talk about current levels of non-prescribed drug and alcohol use and the triggers for use.
- Find out if the individual would like to cut back or eliminate non-prescribed drug and alcohol use.
- Together, develop a plan to cut back or eliminate all drug and alcohol use in the upcoming week.
- Ask the individual to commit to carry out their plan to cut back or eliminate non-prescribed drug/alcohol use in the upcoming week.
