Coaching For Activation®

Involving individuals in their own care is widely recognized as critical to better health, improved care, and the more efficient use of healthcare resources.

Insignia Health’s Coaching for Activation® (CFA) program applies a decade of PAM® research to help health coaches tailor support based upon a person’s self-management ability. Behavioral activation, and the attainment of guideline health behaviors, is achieved through delivering PAM level-appropriate information, setting realistic goals and planning achievable action steps.

With CFA-guided support, individuals steadily will build their knowledge, skills, and confidence over time. Gains in health activation reflect increased positive self-care behaviors that improve the individual’s quality of life and decrease utilization of high-cost health resources.

CFA Goals by PAM Level:

Level 1: Individuals become self-aware of their behaviors and symptoms.
➧Focus coaching on the relationship between symptoms and behaviors, building self-confidence and awareness. Guide individuals in choosing where to start, taking small steps toward highly attainable goals.
Level 2: Individuals begin to develop the knowledge, skills and confidence needed to master new self-management competencies.
➧Focus coaching on helping individuals learn to monitor symptoms and behaviors that can adversely affect attaining goals.
Level 3: Individuals initiate new health promoting behavior(s) and work to further refine techniques to monitor and adjust.
➧Focus coaching on providing encouragement and problem solving. Adjust or add goals as needed. Recognize and celebrate successes.
Level 4: Individuals strive to maintain desired health-related behaviors over time and learn to anticipate difficult situations that will arise.
➧Focus coaching on anticipating and recognizing future issues that may make it hard to stick to a plan and to maintain successful behaviors.
