Jointly brainstorm and problem solve around issues that arise in maintaining newly adopted positive healthy behaviors (like smoking cessation and reducing other smoke exposures)
Set progressive goals where the individual has experienced success and relief of symptoms
Continue with smoking cessation program (if applicable)
Goal: Discuss what opportunities or new self-management behaviors the individual might wish to address to improve their health and successful management of their COPD
Possible Action Steps
Elicit individual’s concerns, questions and ideas related to improving their heart health
Jointly problem solve, plan and try new approaches to mitigate any troubling symptoms
Discuss why symptoms are reduced when clinical targets for weight, blood pressure, and salt/sodium intake are achieved
Explore alternative options for managing COPD if lifestyle changes are not enough to provide relief or management of symptoms
