Elicit concerns, questions and ideas related to better managing their CAD.
Find out what area of CAD management concerns the individual the most (diet, medications, smoking/tobacco cessation, stress control, diabetes management, weight control, doctor visits, getting flu & pneumonia vaccinations, etc.)
Together, create a plan for one CAD self-management improvement (cutting back on tobacco use, being active at least 4 days each week, going for a routine doctor’s visit, losing two pounds, etc.)
Discuss the “silent” symptoms of high blood pressure, high blood cholesterol and early diabetes.
Goal: Learn techniques to get back on the CAD control track.
Possible Action Steps
Discuss why it is difficult to adhere to all of the recommendations for CAD self-management.
Have individual identify ways that are likely to throw them off-track in managing CAD.
Discuss relapse of high-risk behaviors (smoking/tobacco/e-cigarettes, high cholesterol/high sodium diet, inactivity, high stress) -- affirm that relapse happens to everyone.
Ask them to identify a few steps they could take to get back on track.
